Please download and complete all 12 pages of new patient package prior to your scheduled appointment time. There is a $50 fee that you will be charged if you do not show up for your visit in a timely manner with all paper work completed or if you cancel your appointment within less than 24 hours of your scheduled visit. It is not our office policy to "overbook" patients and we try to keep your waiting time to a minimum, but this is only possible if you show up on time for your appointments (We recommend that you arrive at our office 30 minutes prior to your scheduled visit for a new patient appointment with all paperwork completed and 15 minutes prior to your scheduled visit for a follow up visit). You will be billed for this "No Call/No Show" fee if we have not received a cancellation call at least 24 hours prior to you scheduled visit, so that we may make your appointment time available for another patient. We do utilize a billing company and collection agency for all outstanding balances that are not paid in a timely manner.
